AlternaCare is a 21 bed unit providing a continuum of healthcare in the community. Delivery of care ranges from non-acute to supervisory of the geriatric segment. Admission may be from home, extended care facilities or acute care facilities. Both short- and long-term stays are available. AlternaCare is a private-pay facility.
Twenty-four hour professional nursing care is provided by the unit director, RN's, LPN's, Nurse Externs and Nursing Assistants. Under supervision, Health Occupation and Nursing students may receive clinical experience on the unit.
It is the philosophy of AlternaCare that residents will be maintained at, or restored to, their optimal level of function. Toward this end, and with a physician's order, treatment may be administered by Franciscan Health Rensselaer 's Rehabilitation Services; which includes Physical, Occupational and Speech Therapies. Care may also be provided by Respiratory Therapists if ordered. Further, diagnostic procedures are aided by the Hospital's Medical Laboratory and Radiology Departments.
Franciscan Health Rensselaer Dietary staff, under the direction of a Staff Registered Dietician, provide regular and therapeutic diets to residents via selective menus.
Recreational activities are developed by an Activities Director who is assisted by Franciscan Health Rensselaer Auxiliary Volunteers.
Visitation and services by clergy of all faiths are encouraged to fulfill the spiritual needs of the residents.
Periodic resident-centered conferences are held to review the overall plan of care with the patient and his/her family.
The Social Services Director serves as an advocate for the AlternaCare patient, and is an integral part of the unit. Discharge planning by Social Services involves utilization of various resources available in the community.
AlternaCare staff recognizes, protects and promotes the rights of each patient, including the right to submit written directives for their health care. AlternaCare cooperates with the Hospital's Home Health Care & Hospice department in providing care for qualifying terminally ill patients.
Quality assurance audits with patient oriented outcomes are an integral function of the unit.
